Case Study: Meg's Client

This client is 28 years old, 130 lbs. (approximately), and 5 ft. 2. She first came to me last year (May 2017) for the following reasons: Suspected SIBO, Lack of help and support from past practitioners, And what she referred to as a "unique case" of PCOS and low T3 syndrome. She also wanted to address that she had not had a bleed in 3 years.
I first had her complete a stool test and MRT, which she followed a protocol for to address the findings. She reported to experience a lot of relief and has also gained her period back within this time, however, I just received her saliva test results back and would like some support with her protocol, plus addressing her current health complaints, which include:
"Originally, three years ago, my T4 to T3 conversion was found to be poor; Taking 3.5 grains of NDT/day got me to feeling great 2.5 years ago, until a year ago, when I began feeling hypothyroid again and gained weight back; recent bloodwork confirmed T3 "pooling," as in my T3 isn't making it into my cells efficiently; this was further confirmed by the higher than optimal serum iron level and very low level of ferritin. Theories include: low cortisol and mercury toxicity due to MTHFR gene SNP Chronically B-12 deficient despite supplementation; theories include: MTHFD1 and MTHR gene polymorphisms active and the daily consumption of metformin Lack of growth of Lactobacillus and Bifidobacterium species in gut per stool testing last year; cause of insulin resistance and pseudo-PCOS, perhaps? Low morning and evening cortisol Extremely high Sex Hormone Binding Globulin (SHBG), even though my period came back after 3 years of complete amenorrhea A recent hs-CRP reading of 5.5, whereas it had been 1.3 two years ago during my euthyroid, but amenorrheic, state Elevated ALT liver enzyme, whereas it been perfectly in the middle of the range two years ago Curious whether my herbal antibiotic/prokinetic/biofilm disruptor protocol last year successfully fixed my suspected, (though not confirmed by an actual SIBO-specifc test), case of SIBO, or not Gall bladder stasis/fat malabsorption; have been taking OxBile as a band-aid; theory: PEMT gene SNP; hoping that liquid riboflavin & PC, in conjunction with PC IV infusions (to start soon I hope), will remedy this Pancreatic exocrine insufficiency in an extreme way; no amount of betaine HCl makes me feel that "burn" that you've mentioned Leaky gut/food sensitivities And symptoms: Weight loss resistance despite eating significantly less, and moving more, than I had been 1 year ago whilst maintaining my weight Lack of stamina Notable, though not extreme, shortness of breath Hard to get up in the morning Digestion still wonky, (very wonky and painful if I eat without taking supplements), whereby I still flip back-and-forth btwn constipation and diarrhea, but normal stool 70% of the time Inflammation seemingly manifests as joint pain, "hot ear," and occasionally, a flushed face, despite having eliminated all known and suspected food and chemical triggers."

History of eating disorders and drug abuse. Her current supplements include:

InfoWars brand Carnivore Digestive Enzymes (High Protease, also great for Keto, and nothing I don't need!) Methylcobalamin B-12, sublingual Betaine HCl w/ Pepsin & Gentian Bitters Pure Vitamin Club magnesium complex Liothyronine/Cytomel: 40 mcg/day Natural Dessicated Thyroid: had been taking 3 grains/day, but have been switching over to a T3-only protocol via my Primary Care physician 2,000 mg Metformin daily."
Of course, I have only suggested a small handful of this very long list. This client researches everything and begins taking supplements on her own based on her research... One of the reasons I'm very overwhelmed with working with her and would love support from our community!
